Illustrated descriptions of Oregon workplace fatalities

Safety Notes were published from 2000 through 2006 in Oregon Health and Safety Resource, an Oregon OSHA newsletter. They are descriptions of fatalities and serious accidents, with illustrations, investigation results, and information on preventing similar accidents. Safety Notes presented are for information, research, or occupational injury prevention only.

NIOSH FACE program concentrates on investigations of fatal occupational injuries. The primary intent of this program is to provide interested users with access to the full text of hundreds of fatality investigation reports. 

CROET OR-FACE is the Oregon Occupational Fatality Assessment and Control Evaluation program. It is a National Institute of Occupational Safety and Health (NIOSH) sponsored research program designed to identify and study fatal occupational injuries. Investigations conducted through the FACE program allow the identification of factors that contribute to fatal occupational injuries. This information is used to develop comprehensive recommendations for preventing similar deaths.
